An implementation evaluation of procedure videos to support nurses undertaking infrequent clinical procedures in acute cardiology

Abstract

Background

Rare procedures in cardiology are challenging; they are often time-critical, lifesaving and carried out with limited resources. Procedures such as pericardiocentesis are infrequent, and nurses may go several years without assisting with the procedure. Currently, most healthcare organisations support their health professionals during rare procedures with manuals containing standard operating procedures (SOPs). However, when faced with an urgent procedure that has not recently been encountered, healthcare professionals will access whatever information is available, which is increasingly online videos.

Aims

The main purpose of this thesis is to understand the acceptability and use of locally tailored procedure videos. The research aims were to:

Determine the barriers and facilitators to implementing locally tailored clinical procedure videos at two hospitals in NSW, Australia

Design a plan to implement clinical procedure videos using behaviour change theories

Use website data, analytics, and user feedback surveys to understand the usage and acceptability of the videos during the implementation period.

Conclusion

The study shows that the rigorous methods and frameworks used to implement videos have resulted in high uptake. The findings from this study have several practical implications that can help inform future design, methods, and implementation of video interventions. A scoping review revealed that publications related to videos have increased by almost 50% in the past two years, indicating a growing interest in this method of clinical support. The study findings suggest that videos are acceptable, valued, and useful in complementing written procedures. In conclusion, this thesis adds to the emerging body of research demonstrating that videos have the potential to support healthcare professionals during infrequent procedures.
